Payment Options
Dr. Mills has always been committed to offering the best value to his clients, by providing a high level of care at reasonable fees. We are very proud of our fees. They reflect the exceptional level of care, predictability and long-term satisfaction that our clients receive.
Payment is expected at the time of client’s visit. We accept cash, check, debit card, Master Card®, Visa®, Discover®, and CareCredit®.
We know our clients expect us to control costs wherever and whenever possible. Eliminating a billing department with all the personnel and overhead expenses has been the most direct and effective method for controlling costs, thus allowing us to pass those cost savings to our clients, while maintaining our focus on our client’s health, comfort and wellness.
Herbert Mills Dentistry will communicate clearly all recommended therapy and the fees involved prior to beginning therapy.
Insurance is a wonderful supplement to our client’s dental care. Dental insurance is a contract between the client and the insurance company. As a courtesy to clients we file insurance claims electronically the day of their visit and we provide the insurance company with any necessary narratives or x-rays free of charge. We will do everything reasonable to maximize our client’s benefit. Payment for the therapy is due the day of the client’s visit and any reimbursement will be sent from the insurance company directly to the client.
Financial assistance is available through our financial partner, CareCredit®. CareCredit® offers a 3 month or a 6 month interest free program and four extended payment programs. With an extended payment program clients can choose to make monthly payments over 24, 36, 48, or 60 months with a very low interest rate of 9.9%. We can offer our clients a choose of a monthly payment that fits comfortably into their budget.
Clients are also given an opportunity to take advantage of our Courtesy Discounts. A 5% discount is available to clients who pay their therapy in full on the day of diagnosis with cash, check or debit card. A 3% discount is available to clients who pay their therapy in full on the day of diagnosis with Master Card®, Visa®, or Discover®.
At times a client’s therapy may require a Room Prep/OSHA fee of $50.00. The fee is paid at the time the appointment is made. That amount will be applied to the therapy fee on the day of the appointment. We require 48 hours notice to cancel an appointment. If the appointment is canceled less than 48 hours prior to the scheduled appointment time, the Room Prep/OSHA fee will be forfeited by the client.
Clients are asked to sign the Financial/Payment Policy, acknowledging that they understand and agree to Herbert Mills Dentistry’s payment policy.

Insurance Reimbursement
Client’s insurance claims are filed electronically the day of the client’s visit.
Insurance reimbursements are mailed directly to the client from the insurance company.
We know our clients expect us to control costs wherever and whenever we can. By eliminating an insurance billing department with all the personnel and overhead expenses associated with it, we can pass those savings along to our clients and offer quality care at a good value. Clients are still able to use their insurance, we are filing their claims and the insurance reimbursements are sent directly to the client.
We also realize that dental insurance reimbursement levels have gone down the past several years and with most insurance companies the $1000 maximum yearly benefit that was established in 1960 is still the maximum allowed today, over 40 years later.
We offer our clients many payment options and financial solutions; this allows clients the fine care they really want and have it fit within their budget.
We will as a courtesy to clients file their claims electronically and provide the insurance company with any necessary narratives and X-rays; once again working to maximize the client’s benefit.
Deciding on a dental practice is extremely important. What you are essentially investing in is CARE, SKILL, and JUDGEMENT.
Many dental procedures are irreversible.
We truly believe that clients deserve the benefits of vibrant and lasting health, an attractive smile, and the quality of care that Dr. Mills provides. We will do everything reasonable to maximize client’s benefits, getting them as much reimbursement as possible as quickly as possible.

Privacy Policy
Title II of the Health Insurance Portability and Accountability Act of 1996, HIPAA (Public Law 104-191) includes Administrative Simplification (Subtitle F, Part C) which required the establishment of standards for the electronic transmission of certain health information and for the security and privacy of protected health information.
The Standards of Privacy of Individually Identifiable Health Information (The Privacy Rule), promulgated by the Department of Health & Human Services, as directed by HIPAA, provides the first comprehensive federal protection for the privacy of health information by creating standards to protect an individual’s medical records and other personal health information.
At Herbert Mills Dentistry, clients are given the right to review and secure a copy of our Notice of Privacy Practices.
The signed Patient Consent Form authorizes our practice to use and disclose client health information to carry out; treatment, insurance claims, and day to day operations of practice.
The Authorization for Release of Information to Family & Friends identifies persons authorized by clients to receive protected health information and allows clients to limit the type of information released.
The Authorization for Use and/or Release of Information authorizes disclosure of information to other entities and allows restrictions on the information to be released and an expiration date of the authorization.
